A robust improvement in serum total testosterone and LH levels with a decline in oestradiol levels was documented with the use of anastrozole or letrozole over a period of six weeks to six months. Advanced experimental studies and some clinical observations reveal the importance of blocking both the genomic and non-genomic activities of the estrogen receptor, as well as its crosstalk with growth factor and other cellular signaling, for greatest effectiveness of endocrine therapy. Consequently, these studies provide a mechanistic explanation for the superb performance of aromatase inhibitors, and also suggest how inhibiting selected growth factor receptors might delay or prevent the onset of resistance to aromatase inhibitors and other endocrine therapies. Our study suggests, however, that response rates may be lower than previously reported. Data on optimal duration of treatment is needed, but the rarity of LGESS is an obstacle to conducting large clinical trials. Subsequent to IRB-approvals from two medical centers and the Cancer Registry of California, women were recruited with flyers and mailed invitations in urban and suburban areas of South California.
